MODERN FLORAL DESIGN--
Complete courses in professional floral
design are concentrated into three weeks, two
weeks, or one week classes. Creative designing for the beginner or the
established florist.
Advance instruction in the newest ideas as well as basic
fundamentals of floral design.
Valuable information on flower shop management, buying, mark-up, sales promotion and floral keeping quality are covered,
but the emphasisis always upon practical floral designing.
We are the only Chicago floral design school approved by the ILLINOIS STATE BOARD OF EDUCATION--this protects YOU and your tremendous INVESTMENT of time and money.

TUITION FEES -
$1,500.00 for the three week class, $1,000.00 for the two week class, $500.00 for the one week class, which includes the very amply quantity of fresh flowers, foliages,
and all the other materials and 2 floral design textbooks used in classroom practice. Application fee of US $50.00 is payable with the application.
TOTAL INSTRUCTIONAL COSTS: $1,500.00 (3 weeks), or $1,000.00 (2 weeks), or $500.00 (1 week).PAYMENT SCHEDULE –
I submit U.S. $50.00 as an application fee to hold a place in the class, which is part of the total. The balance of the tuition, $1,450.00 (or $950.00 or $450.00)
is payable in advance or is payable on the first day of class.
Payments may be made by: cash, check (U.S. bank), or traveler's checks (in U.S. dollars.) CREDIT CARDS ARE NOT ACCEPTED. There are NO additional charges for books or materials.

BUYER'S RIGHT TO CANCEL--The buyer has the right to cancel this contract until the midnight of the 5th business
day after this contract is signed by the buyer and the buyer is accepted by the school. Notice of cancellation shall be
orally or in writing to: James Moretz, 634 S.Wabash Ave., Suite 210, Chicago, IL 60605. 312-922-9328.
This is a legal instrument. All 3 pages of the contract are binding. Reading all 3 pages before signing.
3. This contract is binding only when the agreement is accepted, signed and dated by the authorized official of
the school at the school's principle place of business. 4. You are entitled to an exact copy of this agreement
and any disclosure pages you sign. 5. Under the law you have the right to pay the full amount due; there are no
fiance charges. 6. This contract and the school catalog constitute the entire agreement between the buyer and
and the school. 7. Any changes in this agreement shall not be binding on either the student or the school unless
such changes have been approved in writing by the authorized official of the school and by the student or the
student's parent or guardian, if the student is a minor. 8. The terms and conditions of this agreement are not
subject to amendment or modification by oral agreement.
REFUND POLICY--1. If the prospective student cannot attend the scheduled class, the application fee of $50.00
will be refunded within 2-3 days or held for a later enrollment, provided the school is notified by phone at least
5 days before the class begins. 2. All monies paid by the student will be refunded if the class is canceled or if the
student is not accepted by the school. 3. A refund of all tuition and enrollment fees is to be paid within 30 cal-
endar days after the determination of non-acceptance is made. 4. Application/registration fees shall be charge-
able at initial enrollment and shall not exceed $150.00 or 50% of the cost of the tuition, whichever is less. 5.
Deposits or down payments shall become part of the tuition. 6. The school shall mail a written acknowledge-
ment of a student's cancellation or written withdrawal to the student within 15 calendar days of the postmark
date of notification. Such written acknowledgement is not necessary if a refund has been mailed within the 15
calendar days. 7. All student refunds shall be made by the school within 30 calendar days from the date of
receipt of the student's cancellation. 8. A student may give notice of cancellation to the school in writing. The
unexplained absence of a student from a school for more than 15 days shall constitute constructive notice of
cancellation to the school. For purposes of cancellation, the date shall be the last day of attendance. 9. A school
may make refunds which exceed those prescribed in this Section. 10. A school shall refund all monies paid to it
in any of the following circumstances: a. the school did not provide the prospective student with a copy of the
student's valid enrollment agreement and a current catalog or bulletin; b. the school cancels or discontinues
the course of instruction in which the student enrolled; c. the school fails to conduct classes on days or times
scheduled, detrimentally affecting the student.
11. When notice of cancellation is given before midnight of the 5th business day after the date of enrollment but prior
to the first day of class, all application/registration fees and tuition shall be refunded to the student. 12. When notice of
cancellation is given after midnight of the 5th business day following acceptance but prior to the close of business of
the student's first day of class attendance, the school may retain no more than the application/registration fee which may
not exceed $150.00 or 50% of the cost of tuition, whichever is less. 13. When notie of cancellation is given after the
student's completion of the first day of class attendance, but prior to the student's completion of 5% of the course of
instruction, the school may retain the application/registration fee, an amount not to exceed 10% of the tuition or $300,
whichever is less. 14. The school may retain an amount computed prorata by days in class plus 10% tuition up to
completion of 60% of the course of instruction. 15. When the student has completed in excess of 60% of the course
of instruction, the school may retain the application/registration fee and the entire tuition. 16. A student who on personal
initiative and without solicitation enrolls, starts, and completes a course of instruction before midnight of the 5th business
days after the enrollment agreement is signed, is not subject to the cancellation provisions of this Section.
STUDENT PROGRESS--In order to graduate and receive a diploma from any of the 3 courses, 1. a student must
complete the required daily design projects and 2. may not miss more than 2 days (or the equivalent, during the 3-
week class) or 1 day (or the equivalent, during the 2-week or 1-week classes). A student who is absent for more
than this period of time must make-up the absence in a future class, before receiving a diploma. The American Floral
Art School uses a pass/fail evaluation in the awarding of diplomas. Practical exercises are given each day; students
receive daily evaluation and critique. Written tests are also given and students receive test scores, although no
numerical grades are given on creativity exercises.
STUDENT TRANSCRIPTS--Students may write or call the school for their student record; it will be mailed or
can be picked up.
STUDENT COMPLAINTS--If a student has any problem with the school's curriculum, staff, or tuition, they should
call or wite the school's director, immediately. The director will attempt to resolve the student's problem(s) within 1
or 2 business days. If that is not possible, the student will be referred to the State Board of Education. COMPLAINTS
AGAINST THIS SCHOOL MAY BE REGISTERED WITH THE STATE BOARD OF EDUCATION: 100
